Use this form if you are an employee or an employees鈥� representative and want to make a complaint that an employer:

  • has failed to provide information to determine the number of people employed by the undertaking in the UK
  • has provided information that is false or incomplete

You should make your complaint to the Central Arbitration Committee under Regulation 6(1) of the .

If you are an employer, the CAC will ask you to respond to a complaint made by an employee or employees鈥� representative.
