To verify your child for the childcare service, you will need to use the Child Verification form (CVF1).
Email HMRC to聽ask for this form in Welsh (Cymraeg).

You will fill this form in online and you cannot save your progress. It is best to get all of your information together before you start.

Documents you need to send with the form
If your child was born in the UK, you will need to send either the child鈥檚:
- birth certificate
- adoption certificate
If your child was born outside the UK, you will need to send the child鈥檚:
- birth certificate
- passport or travel documents used to enter the UK
Do not send photocopies.
