Child Trust Fund and Junior ISA: terminal illness early access
Use this form if your child is terminally ill and you want to take money out of their Child Trust Fund or Junior Individual Savings Account (ISA).

The terminal illness early access form should be completed by the person responsible for managing the account.
The child can claim if they鈥檙e:
- over 16
- managing the account
You鈥檒l need to get the child鈥檚 medical practitioner to complete the early access medical report form if you are required to provide evidence that your child is terminally ill.
Send the early access medical report form (if applicable) with a copy of your terminal illness early access form to the address shown on the form.
Email HMRC to ask for this form in Welsh (Cymraeg).
