  • argraffwch un o鈥檙 ffurflenni hyn (defnyddiwch y fersiwn Pensiwn y Wladwriaeth os ydych yn cael Pensiwn y Wladwriaeth)
  • llenwch y ffurflen gyda beiro
  • anfonwch at y cyfeiriad sydd ar y ffurflen

Nid oes modd i chi gwblhau鈥檙 ffurflen gan ddefnyddio ff么n symudol neu lechen. Rhaid i chi naill ai::

  • argraffu鈥檙 ffurflen a鈥檌 chwblhau 芒 llaw

  1. Updated versions of the Welsh Carer's Allowance claim form, DS700W Notes, Welsh Notes about Carer鈥檚 Allowance claim form, DS700SPW and Welsh Notes about Carer鈥檚 Allowance claim form notes, DS700SPW Notes have been added to the page.

  2. Updated the English versions of the following forms: 'Carer's Allowance claim form (DS700)', 'Notes about claiming Carer's Allowance (DS700 Notes)', 'If you get a State Pension: Carer's Allowance claim form (DS700SP)' and 'If you get a State Pension: Notes about claiming Carer's Allowance (DS700SP Notes)'. The updated versions are dated 12/24.

  3. Information for Scottish citizens updated and a link provided to Carer Support Payment.

  4. Carer鈥檚 Allowance information, new claims pilot, has been updated to include a further seven areas.

  5. Updated Carer Support Payment (CSP) pilot areas in Scotland to include: Angus, North Lanarkshire or South Lanarkshire.

  6. The Carers Allowance claim forms and notes have been replaced with updated versions following this years uprating.

  7. Added new versions of the Carer's Allowance claim forms. If you live in Dundee City, Perth and Kinross or the Western Isles, you need to apply for Carer Support Payment from the Scottish Government instead of Carer鈥檚 Allowance.

  8. Updated the Carer's Allowance claim form, Notes about claiming Carer's Allowance, If you get a State Pension: Carer's Allowance claim form and also If you get a State Pension: Notes about claiming Carer's Allowance.

  9. Notes have been updated to show changes are to the weekly rate/weekly earnings limit. The forms are unchanged.

  10. Revised DS700 Carer's Allowance form, print, interactive and notes for both (English).

  11. Added revised DS700 and DS700 notes: Carer's Allowance (Welsh).

  13. Published the latest versions of the English claim form to apply for Carer's Allowance if you get State Pension and the accompanying notes booklet.

  24. Added updated claim forms and notes in English and Welsh. The phone numbers have been updated to the 0800 freephone number and the permitted earnings amount has been updated to match the 2018 to 2019 rate.
