- the 11-digit number from the vehicle log book (V5C)

There are other ways to apply for a replacement MOT certificate if you cannot use this service. What you can do will depend on the type of vehicle you have.
Cars, motorcycles and vans
You can ask for a replacement certificate in person from any MOT centre. It does not have to be the centre that did the original MOT.
You鈥檒l need to bring the vehicle registration (number plate) and log book (V5C) reference number. The centre can charge you up to 拢10.
Heavy goods vehicles (HGVs) and trailers
You can fill in and post a document replacement form for HGVs and trailers (VTG59).
You鈥檒l need to pay 拢13.
Public service vehicles (PSVs)
You can fill in and post a document replacement form for PSVs (VTP6).
