Enteric fever online questionnaire instructions

Step 1: Access the online questionnaire
Open your web browser and navigate to the (Figure 1).

If you are starting a new questionnaire, select the box labelled 鈥楥heck if you have no code鈥� and then 鈥楽ubmit鈥�.
You will be directed to the beginning of the online questionnaire.
Step 2: Complete the questionnaire
Each online questionnaire is assigned an Anonymous Login Code (ALC) on the first page. Figure 2 shows the ALC circled in red, which is displayed at the top of the page. It is vital that you make a note of the ALC for each case as this code can be used to resume the questionnaire at a later date.
You may also want to copy and paste this ALC into the Events section of the case鈥檚 HPZone record so that other colleagues can access and edit it if needed.
Amendments to questionnaires can only be made within 30 days of submission.

Please note that completed questionnaires will automatically be sent to the local Health Protection Team (HPT), if the required field is filled in, and the UK Health Security Agency (UKHSA) Travel Health and International Health Regulations (IHR) team. If you require a copy of your completed questionnaire, please contact [email protected] or your local HPT.
Please make sure the 鈥楽ave鈥� button is clicked before selecting 鈥楴ext鈥� for pages 1 and 2 only. This will ensure the details on the questionnaire are saved online.
If you wish to pause and continue a case questionnaire at a later date, select 鈥楽ave鈥� before clicking the 鈥楥ancel鈥� button (Figure 2). This will direct you to the end page. You should exit the end page by clicking on 鈥楲ogoff鈥�, otherwise you may not be able to start a new questionnaire.
When you are ready to resume the case questionnaire, you should follow the instructions to log in provided in Step 1 (Figure 1). When you reach the anonymous login webpage, you should enter the ALC before selecting 鈥楽ubmit鈥�.
Please note that questionnaires will be saved online for a maximum of 30 days.
The final page of the questionnaire has a 鈥楧one鈥� button. Only click on the 鈥楧one鈥� button if the questionnaire has been fully completed.
An email of the completed questionnaire will be automatically sent to the email address provided and to the Travel Health and IHR team (Figure 3).

Figure 3 shows an example questionnaire email with completed fields for a test submission. The email contains:
- the questionnaire title
- responses to section 1. Administration details
- some responses to section 2. Case details
The questionnaire emails will be numbered in the same format as the enteric fever enhanced surveillance questionnaire Word document. You may forward the automated email securely to other relevant public health professionals who require a copy.
You can then save the email or copy the information into a Word document and upload it to HPZone if you wish to add it to the case record.
