OVb37: Sam declaration form

Register a new practice or individual on Sam, the Animal and Plant Health Agency's (APHA's) online system for recording tuberculosis (TB) test results.

Details

This form must be completed by the principal or partner of the veterinary practice. You鈥檒l be asked to give:

  • the practice name, VAT registration number and address
  • the name, SP number and role of all individuals you want to register
  • the previous and current practice names for existing Sam users

Once processed, the practice and any individuals listed on the form will be registered on Sam.

You must tell APHA immediately if an individual鈥檚 access to Sam needs to be withdrawn.
