Use this form to tell the Animal and Plant Health Agency (APHA):

  • that you鈥檝e moved to a new practice, if you鈥檙e an OV or ATT
  • about a change to a practice鈥檚 details, such as name or address

Once your form has been processed, APHA鈥檚 OV team will update all relevant details on Sam, APHA鈥檚 online system for recording tuberculosis (TB) test results.

APHA uses these details to send you important information about your OV authorisation. It鈥檚 your responsibility to make sure the details are correct.
