This Order is made in relation to a complaint that Dylan Quail, in his role as supervisor of an Individual Voluntary Arrangement (鈥業VA鈥�), breached the fundamental principle of Professional Competence and Due Care of the Insolvency Code of Ethics, when he took approximately 15 months to issue a certificate of completion following the full and final settlement of the IVA.
