Defnyddiwch y ffurflenni hyn i wrthwynebu i rywun gofrestru atwrneiaeth arhosol (LPA) neu atwrneiaeth barhaus (EPA).

I wrthwynebu cofrestru LPA, defnyddiwch:

  • LPA006 os chi yw rhoddwr yr LPA ac rydych eisiau gwrthwynebu am unrhyw reswm
  • LPA007 os ydych yn atwrnai neu鈥檔 鈥榰nigolyn i鈥檞 hysbysu鈥� ac rydych eisiau gwrthwynebu ar sail ffeithiol
  • LPA008 os ydych yn unrhyw un (gan gynnwys atwrnai neu 鈥榰nigolyn i鈥檞 hysbysu鈥�) sydd eisiau gwrthwynebu am resymau eraill (sef 鈥榮ail ragnodedig鈥�)

Os ydych eisiau gwrthwynebu ar sail ragnodedig, mae鈥檔 rhaid ichi hefyd a thalu ffi o 拢400.

I wrthwynebu cofrestru EPA:

  • defnyddiwch ffurflen EP3PG os rhoddwyd gwybod ichi am y cofrestru
  • os na roddwyd gwybod ichi

I wneud cais am unrhyw ddogfen mewn fformat arall, megis Braille, recordiad sain neu brint bras, anfonwch e-bost i [email protected]. Dylech gynnwys eich cyfeiriad a鈥檆h rhif ff么n a theitl y ddogfen y mae arnoch ei hangen.
