Only use these forms if the court issued your application before 6 April 2022. If your application was issued on or after 6 April 2022, you will need to use a different form.

Submit one of these forms with your copy of form D84. You cannot apply to dissolve a civil partnership on the ground of adultery. Read more about marriage, civil partnership and divorce.
