A request for details of 鈥榤edical incidents鈥� that led to patient deaths at Brighton and Sussex University Hospitals NHS Trust, Western Sussex Hospitals NHS Trust and East Sussex Healthcare NHS Trust in 2015/16.

NHS Improvement held information on patient safety incidents where the reported degree of harm was death and decided to release it.
