How to complete your form SR01

Follow these instructions to complete your SR01. We鈥檒l reject your form if any of these sections are incorrect, or if you have not paid the correct fee amount.
Part 1: Applicant鈥檚 details
Section 1: Applicant鈥檚 name
Complete the name of the person applying. You should also include any former names.
Section 2: Applicant鈥檚 date of birth
Give the applicant鈥檚 full date of birth.
Section 3: Email address
Give an email address we can use to contact you about the application.
Part 2: Address details on the public register
Section 1: Home address to be removed
Show the home address that you want to remove from the public register.
Section 2: Replace with service address
If you鈥檙e a current serving officer, you must give a new address that you want to replace your home address.
If the address is currently the company鈥檚 registered office, you will need to change your registered office address before completing the SR01.
You do not need to complete this section if you鈥檙e no longer a serving officer, or if the company is dissolved.
Part 3: Documents on the public register
Section 1: Document details
Enter the details of the documents that show the home address.
You must give the document type or the number and title of the form and the registration date. The registration date is the date that we registered the document.

You can only remove your home address from the list of documents Companies House can suppress.
If your home address was used as the company鈥檚 registered office
You must list the original filing which gave your home address as the registered office. This could have been on incorporation or a previous change of registered office address.聽
Do not list the filing that changed the registered office from your home address to a different address, or we鈥檒l reject your application.
Part 4: Authentication
Enter your printed name. You do not need to include a signature.
Part 5: Presenter contact details
Give an address we can use to contact you if your application is rejected.
How to pay the fee
You must pay the correct fee using 188体育 Pay before sending your SR01 application.
It costs 拢30 to remove an address. You鈥檒l need to pay for each document listed in your application.
We鈥檒l reject your application if you do not pay the correct fee amount.
