Do not use this form to change an owner鈥檚 name and/or address, use form TM21A 鈥楥hange of owner鈥檚 name, address or email鈥�.
The Form TM26(R) cannot be used to amend the mark text or change/remove the image. In order to have a mark with a different text/image, a Form TM3 and appropriate fee must be filed for a new application.
