You only need to tell DVLA if your d茅j脿 vu is related to seizures or epilepsy. Fill in form FEP1 and send it to DVLA. The address is on the form.
Bus, coach or lorry licence
You only need to tell DVLA if your d茅j脿 vu is related to seizures or epilepsy. Fill in form FEP1V and send it to DVLA. The address is on the form.
